Florida Hospital Memorial Home Health ensures the smooth transition from hospital to homecare by providing an on-site coordinator at each hospital.  These coordinators work with the physician, patient and family to arrange appropriate post-hospitalization services.  Their role is to coordinate care to ensure no gaps occur at the time of hospital discharge.  They coordinate respiratory, equipment and infusion services, verify what is covered by your insurance, obtain physician orders and communicate vital information to the home care staff. 
Your physician will decide on the best course of action for your care, which may include any combination of services.  The focus of our service is to teach the patient or a caregiver how to safely provide care and manage the disease process.
