Social Services

The social worker will assist you with long-term planning, identify community resources available to meet your needs, assist in identifying social factors impeding recovery and coping mechanisms to assist in rehabilitation, assistance with living arrangements and finances.   Our social workers are Master’s prepared and can assist with:

• Long range planning, advanced directives, care options
• Out-of-home placement assistance
• Addressing safety or sanitation hazards
• Patients who are alone and at risk
• Assistance in accessing community resources
• Discharge planning when follow-up care is needed
