Diabetes Telehealth Program

A customized Diabetes Telehealth program is being released in the spring of 2007!  This chronic disease management program is intended to assist the patient with Diabetes to improve blood glucose control, reduce or prevent complications and promote behavior change.  Through use of the Health Buddy, the patient monitors their blood sugar daily and participates in learning better ways to manage diabetes.

Diabetes CAN be controlled and complications CAN be prevented!  This program was developed utilizing guidelines from the American Diabetes Association and the American College of Endocrinology, as well as our own Certified Diabetes Educator.
